 i beg to state that i lost my SIM along with my <YOUR MOBILE MODEL NO> the
IMEI number of the phone is IMEI no- XXXXX XXXX XXXX yesterday <FULL DATE  i.e D-M-Y> when i was comming from <YOUR LOCATIONG> by <BIKE,CAR,BUS>.when i reached my home i saw
thattheir was no mobile phone in my pocket. My mobile number is <MOBILE NUMBER>
<SIM NUMBER>.

    So, i requested to loage a general diary and take a necessary step. i'll oblige you

Note:  Always carry 1 Xerox copy of purchased Mobile bills and also 1 Xerox copy of application letter

When I went  to Police station for complaint about my phone number and SIM card they asked me how to they know that the stolen/lost mobile was yours that time I’ve shocked that how to I proof that the stolen/lost mobile was mine. Our city officer in charge asked/show me to Mobile Bill, and then I said that yes I’ve bill in home, and then suddenly I back to home and find the purchased mobile bill. And then Xerox that purchased bill with FIR application submits on our police station. They accept my report and they have also given me a copy of my complaint application.

This police complaint letter also works for stolen/lost Bike/car and other stuffs. You just have to change/edit subjects which are given in this letter. If you have lost your Bike/car then don’t forget to mention CAR number with CAR Model (I.e. –TATA NANO, Bajaj Pulsar Bike 220 CC etc.)

Note 2: always try to write complaint letter in simple words.
When I’ve submit my complaint letter I don’t think they read properly but they read my letter A-Z what you actually you have written in your application. So this is very important part of writing complaint letter.
